Personal medication form available online



A simple-to-use form to help you keep track of your medications, plus have the information handy for healthcare providers and other caregivers, is now available on the Baptist Hospital East Web site.

The form allows you to keep a list of your current medications. The list may be used when you visit your doctor or when an emergency occurs. In order to safely take care of a patient, EMS and the Emergency Department need an up-to-date list of all current medications, dosages and frequency (how many times a day the patient takes it) and allergies.

This quick reference guide to your medications is also handy to share with your physician, and if you are admitted to the hospital.
