Heart failure Appropriate Care Measure (ACM)
Heart Failure (HF) is a weakening of the heart’s pumping power. With heart failure, your body doesn’t get enough oxygen and nutrients to meet its needs. The Heart Failure ACM is comprised of 4 measures that reflect standards of care for which there is national support and that have been selected by CMS for public reporting.
The four Heart Failure measures are:

| HF-2 | Left Ventricular Systolic (LVS) Function |
| HF-3 | Angiotensin Converting Enzyme Inhibitors (ACEI) or Angiotensin Receptor Blockers (ARB) for Left Systolic Ventricular Dysfunction (LVSD) |
| HF-4 | Adult Smoking Cessation Advice/Counseling |
